Dear Parents and Guardians of Brooklyn School Students:

 You will soon be receiving an email with a link to our new online Student Information Form (this is replacing the cardstock copy form that was filled out at the start of each school year). You will be receiving a separate email for each of your students, just click on the link to update or verify the basic student information and emergency contacts for your student.

In order to fill out the on-line Student Information/Emergency Contact Form, we have to have an email address on file for you, as the form is being sent via email from our database. You will not be able to fill out the form on-line without an email.  If you do not receive the email, it is possible that it went to your spam folder, we do not have an email on file for you, or your email needs to be updated. Please contact the main office in each school as soon as possible if you do not receive the email. For those families who do not have an email, please let the office know and a Student Information form will be sent home for you to fill out.

Family households have been ‘linked’ in our database for both the elementary and middle schools which means for families who live in the same household, once you have verified or updated your basic contact information ie: phone numbers, emails, that information will be updated for any other BES/BMS students that live in the same household.  Please note that there are certain fields that are considered mandatory (there will be a red asterisk * next to them) and the form cannot be submitted until all of those fields are filled out.

In order to have accurate and up to date contact information for each Brooklyn student, please complete the on-line form as soon as possible.

Thank you for your patience as we worked with our database vendor to streamline our Student Information/ Emergency contact form.
