New Student Registration

Welcome to

Brooklyn Elementary School!


NEW STUDENT REGISTRATION

A student is not registered until the online registration forms are submitted, the registration documentation has been received, 

and health records are approved by the school nurse.


STEP 1: COMPLETE ONLINE REGISTRATION FORMS


  1. Go to www.brooklynschools.org,  From the top line select “Schools'', then choose “Brooklyn Elementary School”. You will be redirected to the Brooklyn Elementary School home page.  From the top line, then click on “New Student Registration.”

  2. Read the Registration Letter. Then, complete the forms on the left (BES Registration form, Bus Request Form, Language Survey Form, Health History & Release of Records)

STEP 2: SUBMIT REGISTRATION DOCUMENTATION


Brooklyn Public Schools does accept pictures and/or scanned document items that are required for registration. Please be advised that at any time the school may request hard copies of such documents.

  1. Copy of Birth Certificate (Long Form)

  2. Copy of Legal Guardianship (if applicable)

  3. Copy of Health Records (see Step 3)

  4. Copy of Custody Papers (if applicable)

  5. Proof of Residency 

    • Copy of lease/rental agreement/mortgage statement* (MANDATORY)

    • Current utility bill (electricity, phone, cable, gas, etc.)

  6. Income Verification (PK only) in the form of W2 ́s for every working member of the household or tax return for the current year.

  7. Nutrition Survey (PK only).


* If your name is not on the mortgage or lease agreement, a notarized letter from the homeowner or renter, attesting to your residency at the address, will be required.  A scanned copy of a notarized letter will not be accepted. 



STEP 3: SUBMIT HEALTH RECORDS

THE FOLLOWING DOCUMENTS MUST BE COMPLETED PRIOR TO ENTRY INTO SCHOOL:

1. Physical Examination (PE): Must be completed between January 1st and December 31st of the calendar year the child starts school 

  • Pre-school students- physical examination- (including hemoglobin or hematocrit tests, weight, blood pressure and a chronic disease assessment- to be completed within one year prior to the first day of school and annually while enrolled in preschool (Early Childhood Health Assessment Record Form) *often called the “yellow form”*

  • Kindergarten students- physical examination- to be completed within one year prior to the first day of school (Health Assessment Record Form) *often called the “blue or white form”*

  • Out of state/out of country transfer students- physical examination within one year prior to the first day of school

2. Immunizations Record: In accordance with the CT Department of Public Health Requirements
3. 
Health History Form (if not completed in step 1)

4. Medication Administration form for administration in schools- signed by pediatrician and parent (if applicable )
5. 
Bee-sting Allergy/Food Allergy-FARE/ Seizure/Asthma/Care-Plan (if applicable)
