New Student Registration

Welcome to

Brooklyn Elementary School!

Due to the ̈Stay Home, Stay Safe ̈ initiative, we are taking all new student registrations online. We are asking parents or guardians to complete the required registration forms and submit all needed documentation via email. Please submit the required documentation listed below to mccallum@brooklynschools.org. The required forms can be found on the left side of this webpage. Once you have submitted the needed documents and completed all required forms, you will receive a confirmation email. We will contact you if something is missing.

ENROLLMENT WILL NOT BE CONSIDERED COMPLETE UNTIL ALL FORMS ARE COMPLETED AND ALL DOCUMENTS ARE RECEIVED. Class placements will be sent out in mid to late August.

REQUIRED DOCUMENTATION
(to be emailed or dropped off at Central Office)

Birth Certificate (long form)
Income Verification (PK only) in the form of W2 ́s or tax return for current year for every working member of the household
Proof of Residency (electric bill / mortgage statement / rental agreement)

These can be submitted by taking a picture, or scanning the document and emailing it to Mrs. McCallum at mccallum@brooklynschools.org or dropping it off at Central Office.

Recent Physical Exam Immunization Record

These can be submitted by taking a picture, or scanning the document and emailing it to Mrs. Brennan at brennan@brooklynschools.org or dropping it off at Central Office.

(If using an iPhone. go to the notes app that comes standard with the phone. Open a new document by clicking on the box with a pencil in it on the bottom right hand corner. When it opens, click on the camera icon. If you take the picture this way, it will send in a PDF format. This is the prefered way of receiving)
